Q: Which form should be used for a controlled drug requisition?
A: Schedule 1, 2 and 3 controlled drugs can be ordered using the following standardised controlled drug requisition forms:

From 30 November 2015 it is mandatory for all prescribers to issue the standardised form.
Requisitions ordered in non-standard formats must not be supplied. Hospices and prisons are exempted from this regulation.
Supplies cannot be made against faxed or photocopied requisition forms.
Legally controlled drug requisition forms must contain the following details:
- Signature of the recipient.
- Name of the recipient.
- Address of the recipient.
- Profession or occupation.
- Total quantity of drug.
- Purpose of the requisition.
