Your Voice Matters in Medication Safety

Medication errors are commonly associated with oral liquid medicines. 

While its use is generally discussed in the context of children, over 25 million prescription items of liquid medicines are dispensed annually for adults in the community in England. 

Serious incidents, such as the tragic deaths of an infant and an elderly patient due to liquid morphine errors, underscore the risks associated with these medications, particularly during transitions across care settings.

Our Medication Safety Officer, Jacquie Lee, has been asked by the Community Pharmacy Patient Safety Group to reach out to pharmacy teams to help gather data for a patient safety survey.

It is open to any pharmacy team members involved in dispensing oral liquid medicines, not just pharmacists.
