2025/26 Community Pharmacy Assurance Framework (CPAF) screening questionnaire

The 2025/26 Community Pharmacy Assurance Framework (CPAF) screening questionnaire is available for pharmacy contractors to complete for a 4-week period from Tuesday 1st July 2025 to Thursday 31st July 2025.

Contractors must complete the screening questionnaire and, if required, the full CPAF questionnaire as it is part of the Terms of Service within the NHS pharmacy contract.

The questionnaire should be completed on the NHS Business Services Authority (NHSBSA) Manage Your Service (MYS) portal. Any contractors who are not yet signed up for MYS will be able to complete the questionnaire online.

The questions are also published on the NHSBSA website for consideration before entering the answers on MYS. The questionnaire will consist of 10 questions and take approximately 10-20 minutes to complete.

Contractors will receive information and instructions on how to complete the screening questionnaires by email from the NHSBSA and if you have not received an email by Wednesday 2nd July 2025 you should contact the NHSBSA at pharmacysupport@nhsbsa.nhs.uk .

Responses to the questionnaire will help NHS England identify a small number of pharmacies that will be considered for a contract monitoring visit and/or asked to complete a full CPAF questionnaire.
